Book by Phone

Please call us on (08) 6374 2980 or 0400 909 665 to book an appointment by phone. You can email us on [email protected] to make any further enquiries.

At your first appointment you will be asked to fill out a patient details form which will ask questions regarding: contact details, medical history, medications list, and dental history. We have attached a copy of this form which you may print and fill out prior to your appointment if you wish to do so:
